Application forms are available via e-mail. Listed below is a preliminary list of information and documentation needed which will assist us in evaluating your loan request:
- Last 3 years of business financial statements including balance sheet, profit and loss statement, and accountants notes if applicable.
- Corresponding business tax returns (including those of affiliates) for the last 3 years, Federal Only.
- Personal financial statements (SBA form) – to be completed by anyone owning 20% or more of the business.
- Personal income tax returns for the past 3 years on individuals owning 20% interest or more, Federal Only.
Also required are current interim business financial statements, balance sheet, and profit & loss no older than 60 days, and the following attached supporting schedules and forms:
- Business debt schedule
- Family budget
- Accounts receivable and payable aging forms
- Income and expense projections for 12 periods
- Assumptions suggestions to support projects
- SBA loan application
- Management resume
- Brief history of business
- Designing your business plan (for start-ups and new business only)
